Each dot is a provider location with at least one posted cash price. Mail-in labs count everywhere.

On a price
- Provider's own site
- The imaging center, lab or surgeon posted the number on its own page or price sheet. We link the page and keep the exact words.
- Hospital price file
- The discounted cash price from the hospital's machine-readable file, required under 45 CFR 180. Usually the facility charge only.
- Marketplace listing
- Found on a bundled-price marketplace when the provider posts nothing itself, and labeled with the marketplace name.
Who takes cash
- Cash practice
- A specialist, surgeon, imaging center or lab that posts its own self-pay prices or says it runs on cash. We quote the page.
- Medicare opt-out doctor
- On the CMS opt-out list, so they contract privately with patients and set their own prices. Opting out alone does not mean a price is posted.
- DPC clinic
- Membership primary care for a monthly fee. The clinic is where a referral to the other two starts.
